Closed Reduction of Proximal Femoral Anti-Rotation Intramedullary Nail for the Treatment of Intertrochanteric Fractures in Elderly Patients and Its Effect on Early Weight-Bearing After Surgery
Abstract:Objective: To explore the application effect of closed reduction proximal femoral anti-rotation intramedullary nail in elderly patients with intertrochanteric fractures and its impact on early weight-bearing after surgery. Methods: A retrospective analysis was conducted on the clinical data of 82 patients with intertrochanteric fractures admitted to Xiaiyi County Second People's Hospital, Henan Province from March 2020 to October 2022. The patients were divided into a control group and an observation group according to different treatment methods, with 41 cases in each group. The control group received dynamic hip screw internal fixation, while the observation group received closed reduction proximal femoral anti-rotation intramedullary nail internal fixation. The clinical related indicators, Harris hip scores before and 6, 12, 24 weeks after surgery, quality of life before and 6 months after surgery, and the incidence of complications were compared between the two groups. Results: The operation time, hospitalization time, incision time, and fracture healing time of the observation group were shorter than those of the control group, and the intraoperative blood loss was less, with statistically significant differences (x2=0.108, 5.642, 6.469, 8.080, P<0.05). After surgery, the Harris scores of both groups showed a gradually increasing trend, with the observation group being higher, and the differences were statistically significant (x2=13.374, 16.411, 11.019, P<0.05). Six months after surgery, the physical function, psychological function, social role, and material life scores of both groups improved, with the observation group showing higher scores, and the differences were statistically significant (t=13.557, 13.980, 10.873, 12.013, P<0.05). The total incidence of complications in the observation group was lower, with statistically significant difference (x2=6.609, P<0.05). Conclusion: Closed reduction proximal femoral anti-rotation intramedullary nail has a definite therapeutic effect in elderly patients with intertrochanteric fractures, which can effectively shorten the operation time, hospitalization time, incision time, and fracture healing time, reduce intraoperative blood loss, improve postoperative hip joint function, enhance quality of life, and reduce the incidence of postoperative complications, which is beneficial for patient prognosis.
