Best Evidence Summary on Self-Management of Patients Taking Oral Warfarin
Abstract:Objective: To summarize and evaluate the best evidence on self-management of patients taking oral warfarin. Methods: A search was conducted in UpToDate, the Guidelines International Network (GIN), the National Guideline Clearinghouse (NGC), the National Institute for Health and Care Excellence (NICE), the Registered Nurses' Association of Ontario (RNAO), the Joanna Briggs Institute (JBI) Evidence-Based Practice Centre in Australia, the Cochrane Library, PubMed, Web of Science, Embase, CINAHL, Yimai Tong, China Biomedical Literature Database, China National Knowledge Infrastructure (CNKI), Wanfang Database, VIP Database, and the Chinese Society of Thoracic and Cardiovascular Surgery for evidence related to self-management of patients taking oral warfarin. The sources of evidence included clinical decision-making, guidelines, evidence summaries, systematic reviews, and expert consensus. The search period ranged from the establishment of the databases to June 30, 2023. Results: A total of five articles were included, including one clinical decision-making article, one guideline, two systematic reviews, and one evidence summary. A total of 17 pieces of evidence were extracted from four aspects: assessment, health education, self-monitoring, and dose adjustment and follow-up. Conclusion: This study compiled the best evidence for self-management of patients taking oral warfarin, which can provide references for the clinical implementation of self-management assessment, training, anticoagulant monitoring, and follow-up management for patients receiving oral warfarin treatment, thereby improving the safety of anticoagulant therapy and patient satisfaction with medical care.
