Effects of Chronic Illness Trajectory Model (CITM) Nursing on Pulmonary Function, Nutritional Status, Emotional State, and Quality of Life in Patients After Video-Assisted Thoracoscopic Lung Cancer Radical Surgery
Abstract:Objective: To explore the effects of the Chronic Illness Trajectory Model (CITM) nursing on pulmonary function, nutritional status, emotional state, and quality of life in patients after video-assisted thoracoscopic lung cancer radical surgery. Methods: A total of 78 patients who underwent video-assisted thoracoscopic lung cancer radical surgery at Xinxiang Central Hospital from February 2021 to March 2023 were prospectively selected and randomly divided into a conventional care group and a CITM group, with 39 patients in each group. The conventional care group received routine nursing, while the CITM group received CITM nursing in addition to routine care. After a 6-month intervention, compliance, pulmonary function, nutritional status, emotional state, uncertainty about illness, and quality of life were compared between the two groups. Results: The total compliance rate in the CITM group (94.87%) was higher than that in the conventional group (76.92%) (P < 0.05). After the intervention, the CITM group showed higher values in MVV%, total lymphocyte count (TLC), hemoglobin (Hb), albumin (ALB), FEV1% predicted value, and forced vital capacity (FVC) compared to the conventional group (P < 0.05). After the intervention, the CITM group had higher scores in positive mood on the Profile of Mood States (POMS) and all dimensions of the General Quality of Life Inventory-74 (GQOL-74), while showing lower scores in negative mood on POMS and uncertainty about illness on the Mishel's Uncertainty in Illness Scale (MUIS) compared to the conventional group (P < 0.05). Conclusion: Applying CITM nursing to patients after video-assisted thoracoscopic lung cancer radical surgery can significantly improve disease cognition, reduce adverse emotions, regulate nutritional status, enhance compliance, promote pulmonary function recovery, and thus improve the quality of life of patients.
