Effect of preoperative serum uric acid level on the prognosis of patients with acute myocardial infarction after percutaneous coronary intervention

摘要:Background Objective:To investigate the effect of preoperative serum uric acid level on patients with acute myocardial infarction after percutaneous coronary intervention(PCI).Methods An analysis of 1610 patients were conducted retrospectively.The effect of preoperative uric acid level on hospital death and postoperative major cardiovascular adverse events(MACE)was evaluated by building three multiple logistic regression models incor-porating various variables.The trend of preoperative uric acid level on postoperative in-hospital death and MACE was evaluated by constructing restricted cubic spline(RCS).Additionally,various uric acid levels were investigated for their effects on postoperative infections,intensive care times,and hospitalization expenses.Results There were 91 deaths(5.7%)and 158 MACE(9.8%)in our results.After model adjustment,uric acid level was associated with death in hospital(OR:2.29;95%CI:1.23-4.33,P=0.010).The incidence of postoperative MACE was also associated with high preoperative uric acid concentrations and increased with increasing concentrations(OR:2.31,95%CI:1.46-3.67,P<0.001).The incidence of postoperative infection complications in the high uric acid group was higher than low uric acid group(34.4%vs.14.3%,P<0.001).The duration of intensive care and hospitalization costs were also higher in the high uric acid group than in the low uric acid group(P<0.001).Conclusions The prognosis of patients is affected by elevated levels of preoperative uric acid.The monitoring and management of uric acid level before operation should be strengthened.
