退变性胸腰椎/腰椎后凸的研究进展

摘要:Thoracolumbar/lumbar degenerative kyphosis ( TL/LDK ) is a sagittal spinal deformity resulting from spinal degeneration with a manifestation of the loss of normal thoracolumbar/lumbar lordosis. TL/LDK is one of the reasons that account for low back pain and trunk deformity with a prevalence in female geriatric patients. Various factors such as gender, age, unhealthy lifestyle, disc and paraspinal muscle degeneration and so on are closely related to the development of its pathogenesis. Thoracolumbar/lumbar region is always involved during its development and then postures like pelvic retroversion, hip lfexion, knee hyperextension, cervical hyperextension and so on will emerge for its compensatory changes. Patients with TL/LDK are accompanied by old age, osteoporosis, muscle degeneration and other comorbidities. So it is of importance to evaluate comprehensively according to clinical symptoms and radiographic results of the patients, so as to choose an appropriate conservative or surgical method. To realign sagittal balance and prevent complications are the primary considerations in surgery. Thus, it is considered crucial to decide the surgical approach, osteotomy level, upper instrumented vertebra ( UIV ) and lowest instrumented vertebra ( LIV ). In this review, we focus on the research progress on TL/ LDK to get a better understanding of its pathogenesis, classiifcation, compensatory mechanism, management and complications.
