Meta-Analysis and Data Mining of the Clinical Efficacy of Clearing Heat and Cooling Blood Method in the Treatment of Immune Thrombocytopenia
Abstract：Objective: To evaluate the effectiveness and safety of the clearing heat and cooling blood method in treating immune thrombocytopenia (ITP), screen high-frequency drugs and their associations, and explore the medication rules. Methods: First, computer search of major Chinese and English databases was conducted. Literature was selected according to inclusion and exclusion criteria. Then, the Cochrane risk of bias assessment tool and RevMan 5.4 were used for quality evaluation and meta-analysis of the included studies. The traditional Chinese medicine formulas were entered into an Excel data table, and high-frequency drugs were screened. Then, the Apriori algorithm in IBM SPSS Modeler 18.0 was used for association analysis to screen the optimal drug combinations. Finally, a core drug co-line network diagram was drawn using Cytoscape. Results: A total of 17 studies involving 1147 patients were included. The treatment with clearing heat and cooling blood herbs showed better clinical efficacy [RR=1.27, 95%CI(1.19,1.35), P<0.05]; platelet count [MD=36.15, 95%CI(26.14,46.17), P<0.05]; platelet-related antibodies [MD=-21.97, 95%CI(-34.51,-9.43), P=0.05]; and incidence of adverse reactions [RR=0.33, 95%CI(0.15,0.69), P=0.04] compared to monotherapy with Western medicine. Data mining revealed that the high-frequency drugs were Danpi, Sheng Dihuang, Huangqi, Sheng Ganchao, Shui niujiao, Xianhecao, Cebaiye, Lianqiao, and Zizhi. The best drug pair was Sheng Dihuang-Danpi, and the best drug combination was Sheng Dihuang-Shui niujiao-Danpi and Sheng Dihuang-Xianhecao-Danpi. Conclusion: For the treatment of ITP, clearing heat and cooling blood herbs can improve clinical efficacy to some extent, increase platelet levels, and have higher safety. Moreover, their medication rules can provide theoretical basis for clinical application.
