Effect of Tiaoshen Jieyu Combined with Xiaoxingnao Acupuncture on Cognitive Function and Mental Behavior in Patients with Post-Stroke Depression with Anxiety Comorbidity
Abstract:Objective: To observe the effects of Tiaoshen Jieyu combined with Xiaoxingnao acupuncture on neurological rehabilitation, cognitive function, and mental behavior in patients with post-stroke depression with anxiety comorbidity (PSCAD). Methods: A total of 100 PSCAD patients hospitalized at Beijing Municipal Miyun District Chinese Medicine Hospital from January 2023 to December 2023 were selected as the study subjects and randomly divided into a control group and an observation group, each with 50 cases. Both groups received conventional treatment for cerebral stroke and cognitive rehabilitation training. The control group received oral administration of Dailixin, while the observation group received Xiaoxingnao acupuncture combined with Tiaoshen Jieyu acupuncture on top of the control group's treatment. The clinical efficacy, psychological state, neurological function, daily living ability, cognitive function, memory function, executive function, mental and behavioral symptoms, and related serum factors were compared between the two groups. Results: The total effective rate of the observation group (90.00%) was significantly higher than that of the control group (80.00%), with a statistically significant difference (P<0.05). After treatment, the Hamilton Depression Scale (HAMD), Hamilton Anxiety Scale (HAMA), and National Institutes of Health Stroke Scale (NIHSS) scores of the observation group were lower than those of the control group, while the Barthel Index (BI) score was higher, with statistically significant differences (P<0.05). After treatment, the Montreal Cognitive Assessment (MoCA) and Rivermead Behavioral Memory Test (RBMT) scores of the observation group were higher than those of the control group, with statistically significant differences (P<0.05). After treatment, the Neuropsychiatric Inventory (NPI) score of the observation group was lower than that of the control group, and the scores of all dimensions of the Behavioral Assessment of the Deficits in Executive Function (BADS) were higher, with statistically significant differences (P<0.05). After treatment, the levels of serum 5-hydroxytryptamine (5-HT), nerve growth factor (NGF), and brain-derived neurotrophic factor (BDNF) in the observation group were higher than those in the control group, while the levels of neuron-specific enolase (NSE), homocysteine (Hcy), and high-sensitivity C-reactive protein (hs-CRP) were lower, with statistically significant differences (P<0.05). Conclusion: The application of Dailixin combined with acupuncture therapy in PSCAD patients shows significant efficacy, which can improve the patient's psychological state, promote neurological recovery, enhance daily living ability, and improve cognitive function, memory function, executive function, and mental and behavioral symptoms. It can also regulate the secretion of related neurotransmitters and neural cell factors, and inhibit oxidative stress and vascular endothelial inflammatory responses.
