Predictive Value of Ultrasound Carotid Plaque Parameters for Stroke Events in Patients with Transient Ischemic Attack and Carotid Stenosis
Abstract:Objective: To explore the predictive value of ultrasound carotid plaque parameters for stroke events in patients with transient ischemic attack (TIA) and carotid stenosis. Methods: A retrospective study was conducted on the clinical data of 114 patients with TIA and carotid stenosis admitted to Shangcai County People's Hospital from January 2023 to June 2024. The patients were divided into a stroke group (47 cases) and a non-stroke group (67 cases) based on whether they experienced stroke events. All enrolled patients underwent carotid ultrasound examination. The carotid plaque parameters of the two groups were compared, and the predictive value of ultrasound carotid plaque parameters for stroke events was analyzed using receiver operating characteristic (ROC) curves. Results: Among the 114 patients with TIA and carotid stenosis, 47 patients developed ischemic stroke, accounting for 41.23% of the total. The stroke group had significantly higher values in plaque thickness, plaque length, plaque area, plaque volume, resistance index, and maximum flow velocity at the stenotic site compared to the non-stroke group (P < 0.05). Multivariate logistic regression analysis showed that the above ultrasound carotid plaque parameters were independent risk factors for stroke occurrence in patients with TIA and carotid stenosis (P < 0.05). ROC curve analysis showed that the areas under the curve (AUC) for predicting stroke events were 0.812 for plaque thickness, 0.785 for plaque length, 0.798 for plaque area, 0.776 for plaque volume, 0.832 for resistance index, and 0.711 for maximum flow velocity at the stenotic site (all P < 0.05). Conclusion: Ultrasound carotid plaque parameters have certain predictive value for stroke events in patients with TIA and carotid stenosis, and can serve as an important indicator for early identification of high-risk populations in clinical practice.
