Fatal Urinary Tract Infection: A Case of Emergency Emphysematous Pyelonephritis
Abstract:Objective To explore the clinical characteristics and comprehensive management strategies for emergency emphysematous pyelonephritis (EPN) combined with diabetic ketoacidosis (DKA). Methods We retrospectively analyzed the clinical data of a 54-year-old female patient with EPN combined with DKA, and summarized the treatment plan based on abdominal CT, blood and urine culture, and multidisciplinary collaboration. The treatment included percutaneous nephrostomy drainage, meropenem for infection control, glucose regulation, and fluid resuscitation. Results After treatment, the infection indicators [C-reactive protein (CRP), procalcitonin (PCT)], renal function (creatinine), and blood glucose significantly improved. The patient was discharged after 15 days and showed no recurrence during one month of follow-up. Conclusion EPN requires early CT diagnosis and pathogen support, and a combination of minimally invasive drainage and sensitive antibiotics can effectively control the infection and improve the prognosis. Clinicians should be highly alert to the possibility of EPN when diabetic patients develop infections.
