椎小关节滑膜囊肿手术一例报道

摘要：Objective To explore the key clinical diagnostic points and surgical strategies by reporting a case of surgical treatment for facet joint synovial cyst,so as to provide experience for clinical diagnosis and treatment.Methods A retrospective analysis was performed on the clinical data of a patient with facet joint synovial cyst who was diagnosed and surgically treated in the Department of Spinal Surgery,Shandong Provincial Hospital of Traditional Chinese Medicine.Results The patient was admitted on October 5,2024,with limited lumbar mobility,obvious tenderness in the interspinous space of L3-4 vertebrae and paraspinal soft tissues,as well as radioactive pain and numbness in the left lower extremity.Imaging examinations(X-ray,CT,and enhanced MRI)confirmed the diagnosis of space-occupying lesion in the lumbar spinal canal,and the patient underwent surgical treatment under general anesthesia.The surgical plan included total laminectomy and spinal canal decompression at L3-4 segment,simultaneous resection of the intraspinal space-occupying lesion and adjacent intervertebral disc,followed by bone graft fusion and internal fixation.Postoperative follow-up showed that during reexaminations at one month,three month,and one year after surgery,the patient's original low back pain,left lower extremity radioactive pain,and numbness were significantly relieved,and no walking dysfunction occurred during the postoperative rehabilitation period.Conclusions Facet joint synovial cyst is clinically rare,and surgical intervention is the preferred treatment for radical resection of the lesion.Clinically,enhanced lumbar MRI should be performed timely,and attention should be paid to differentiate it from other diseases to reduce the misdiagnosis rate.
